Third-hand Smoke: Growing Awareness of Health Hazard


We know that smoking and secondhand smoke exposure are harmful, but what about “third-hand smoke”? This is a relatively new term used to describe the residual contamination from tobacco smoke that lingers in rooms long after smoking stops and remains on our clothes after we leave a smoky place. It may seem merely like an offensive smell, but it is also indicative of the presence of tobacco toxins.

Third-hand smoke consists of the tobacco residue from cigarettes, cigars, and other tobacco products that is left behind after smoking and builds up on surfaces and furnishings. Tobacco smoke is composed of numerous types of gasses and particulate matter, including carcinogens and heavy metals, like arsenic, lead, and cyanide. Sticky, highly toxic particulates, like nicotine, can cling to walls and ceilings. Gases can be absorbed into carpets, draperies, and other upholsteries. A 2002 study found that these toxic brews can then reemit back into the air and recombine to form harmful compounds that remain at high levels long after smoking has stopped occurring.

You can’t see them, but they’re all around us. They aren’t listed as ingredients on the objects we bring in our home, but they’re often there. They’re volatile organic compounds, or VOCs, a wide range of carbon-based molecules (organic compounds) used in a wide range of products that find their way into our homes.

Under normal conditions, they vaporize, effectively leaving their host and entering the air (that’s the “volatile” part) where they combine with other airborne compounds to form a potentially harmful mixture of chemicals, which isn’t good to breathe.

Volatile Organic Compounds, or VOCs, are harmful chemicals emitted by many of the products and materials we have in our homes. VOCs are chemicals that are dispersed as gases from solids or liquids that evaporate into the air at room temperature. Concentrations of these chemicals can be up to 100 times higher indoors than outdoors. Thousands of products emit these gases while in use or even when stored. Many of these products are used or being exposed on a daily basis. It is also known that most VOCs cannot be detected by smell.

The risk of health effects from inhaling any chemical depends on how much is in the air, how long and how often a person breathes it in. Breathing low levels of VOCs for long periods of time may increase some people’s risk of health problems. Several studies suggest that exposure to VOCs may make symptoms worse in people who have asthma or are particularly sensitive to chemicals. These are much different exposures than occupational exposures to VOCs.


At the present time, there are no national or state standards that are specific for school, office, or home settings. In addition, there are many opinions in the science and medical communities about the degree of risk posed by various amounts of VOCs.

Although much research needs to be performed to establish acceptable exposure levels to mixtures of VOCs that may be present in homes, schools, offices, and commercial buildings; recent European research has provided a good starting point, and provides a basis for the IAQ Index.

In general, the research has found that exposures to typical mixtures of VOCs below 1.0 ppm (1,000 ppb) should not result in health effects or significant occupant complaints, exposures in the range of 1 to 10 ppm should be expected to produce some health effects and complaints, and exposures above 10 ppm may produce more serious health effects.

  • People.  70-90% of what you see floating in a ray of sunshine entering your home is dead human skin flakes. And remember, we can only see about 10% of the airborne pollutants. Dead human skin is a staple of the dust mite diet.
  • Dust.  House dust is a mixture of diverse substances that can cause allergies. House dust is composed of a number of natural substances, including dried food particles, mold spores, pollen, fabric fibers, animal dander, and insect parts, especially those of dust mites and cockroaches. However, particles and debris from dust mites are the major source of allergens in dust. As is the case with other allergens, these particles contain proteins that are small enough to become airborne and inhaled.

Norovirus, the USA’s leading cause of foodborne illness, has become known as causing mass outbreaks of food poisoning – and misery – in restaurants.

Food handlers, such as cooks and waiters, cause about 70% of norovirus outbreaks related to contaminated food, mostly through touching “ready to eat” foods – such as sandwiches or raw fruit – with their bare hands, according to a new report from Centers for Disease Control and Prevention. More than 90% of contamination occurred during food preparation, and 75% of food involved in outbreaks was consumed raw.

Business practices in the food industry may contribute to the problem.

One in five restaurant workers admits having reported to work while sick with diarrhea and vomiting – the two main symptoms of norovirus – within the past year, the CDC says.

About 20 million Americans are sickened with norovirus every year, with a total of 48 million suffering food poisoning from all causes. The highly contagious family of viruses also causes up to 1.9 million doctor visits; 400,000 emergency room visits; up to 71,000 hospitalizations; and up to 800 deaths, mostly in young children or the elderly. Infections cost the country $777 million in health care costs.

Norovirus is wildly contagious.

As few as 18 viral particles can make people sick. In other words, a speck of viruses small enough to fit on the head of pin is potent enough to infect more than 1,000 people, according to a CDC report. The virus can spread rapidly in close quarters, as well, such as dormitories, military barracks and nursing homes.

“Norovirus is one tough bug,” said CDC director Thomas Frieden.

Norovirus can make people violently ill so quickly that they don’t have time to reach a bathroom, says Doug Powell, a food safety expert in Brisbane, Australia, and author of People who get sick in public often expose many others. Norovirus also can live on surfaces, such as countertops and serving utensils, for up to two weeks.

Norovirus is also the Terminator of germs — very tough to kill. Alcohol-based hand sanitizers don’t work very well, says Lee-Ann Jaykus, a professor of food science at North Carolina State University. That makes frequent handwashing important.
